w takim wypadku jeśli wszystko w modyfikowanym htaccess jest w porządku,
można spróbować trzy rzeczy
- prawa dostępu htaccess
- restart apache
- sprawdzić kodowanie pliku htaccess (powinno być utf-8)
W moim wypadku błędem było kodowanie utf-8 z sygnaturą
